Please enter the physician's last and first name
and the password you have been assigned.
The information will appear and can be printed for your records. If you feel that
any of the information is incorrect, please notify the Medical Staff Office immediately.
We hope this service will expedite your requests for staff verification.
If the physician is inactive, former, etc., membership and privileges of those physicians must be requested in writing and addressed to the Medical Staff Services Office, 1225 North State Street, Jackson, MS 39202.
Unless otherwise stipulated in writing by the physician, the release of information signed by the physician at appointment or reappointment covers the verification of this information on-line. (approved MEC 1/11/00)
